Integrated Health Services

Strategic objective is to increase access to integrated health care services for children, women and men in counties of focus by 30% by 2026.

AFOD efforts towards improving the health services in South Sudan are informed by the country’s poor state of health services. The key issues in the South Sudan Health Sector include; A double burden of communicable and non-communicable diseases; Low access to and coverage of basic health care services especially in hard-to-reach communities due to capacity gaps; Inadequate and inequitable distributed human resources, both technical and administrative, from the national to the state, county, and Payam levels; Inadequate financial and material resources to increase access to and coverage of services; Unequal distribution of health services with an urban bias; to the disadvantage of rural communities; Frequent shortages of drugs in public hospitals and pharmacies coupled with non-affordability of medicines from private pharmacies by the vast majority of the populace and Weak health management and administrative support systems including system-wide planning, monitoring and evaluation systems.

The strategic intervention areas under this program component include:

  • Control of communicable Diseases (malaria, HIV/AIDs and Tuberculosis control)

Strategic Objective: To increase access to integrated health promotion, disease prevention, and curative services for children, women, and men in counties of focus.

  • Build the capacity of boys and girls on reproductive health.
  • Support rehabilitation of PHC facilities to create space for service.
  • Build the capacity of health workers to provide quality integrated MCH services.
  • Support advocacy and community education on RH using locally appropriate media.
  • Strength community referral systems through provision of transport and communication equipment.
  • Support integrated MCH out-reaches services.
  • Support state and county health departments to conduct quarterly support supervision.
  • Strengthen the capacity of county health departments to undertake RCH data collection, analysis, dissemination and use, including operational Research.

Child Health Subcomponent

Strategic Objective:   Increased access to and demand for quality child health services in the counties of focus.

Expanded programme on immunization.

  • Support micro-planning meetings for EPI at national and state campaigns at the county.
  • Provide support for the distribution of vaccines, cold chain supplies, and maintenance of cold chain.
  • Support community advocacy campaigns and awareness creation to improve vaccination coverage.
  • Provide support to integrated EPI outreach and mobile services to hard-to-reach.
  • Production and distribution of IEC materials for
  • Support supervision and monitoring of immunization.
  • Support data management, quarterly and annual reviews of immunization program performance at county and state
  • Support for EPI operations.

IMCI/IC CM Program subcomponent

Strategic objective. Increase children’s access to treatment against malaria, pneumonia and diarrhea within 24 hours of disease onset.

  • Build the capacity of health service providers in IMCI /ICCM service.
  • Support production and dissemination of IEC/BCC materials and messages
  • Support integrated community clinical outreaches.
  • Facilitate Quarterly support, mentorship and supervision of IMCI services.
  • Support quarterly IMCI review meetings.
  • Support training on HMIS and data management for health

Control of communicable Diseases Program component

Strategic objective: To reduce the prevalence and incidence of communicable diseases, especially Malaria, HIV and Tuberculosis within the communities.

HIV/AIDS Control Health Program

Strategic Sub-objective: To increase access to and demand for HIV/AID prevention, treatment, care, and support services.

Malaria sub programme

Strategies and activities

  • Build capacity of facility and community-based health workers in case management of malaria disease.
  • Strengthen capacity of laboratory staff in malaria diagnosis using microscopy and RDTs
  • Build the capacity of Boma Health Teams (BHT) on home-based management of fever.
  • Facilitate Community Volunteers/VHTs with supplies for home-based management of fever.
  • Support community mobilization and education on malaria prevention
  • Support other malaria prevention interventions e.g., IPT at MCH clinics, IRS.
  • Support integrated support supervision of malaria control services.
  • Facilitate Operational Research on Malaria.
  • Support County health departments in weekly reporting and monitoring of cases of fever.

HIV/AIDS control Health Program

Strategic Sub objective: To increase access to and demand for HIV/AID prevention, treatment, care and support services.

Strategies and activities

  • Facilitate community dialogue meetings on HIV/AIDS prevention.
  • Support community mobilization on HIV/AIDS to create awareness and demand for services.
  • Build capacity of health workers on HIV counseling and testing skills
  • Provide community HIV services including HTS, ART delivery, EID and VL testing services.
  • Support capacity building of health workers and provide mentorship in Option B+ service delivery.
  • Support MOH/CHD to to manage HIV/TB program.
  • Build the capacity of health workers on integrated HIV/AIDS and TB collaborative treatment and care.
  • Establish & support activities of mother-to-mother support groups for PMTCT/MNH
  • Support distribution of HIV/AIDS
  • Support state and county coordination mechanisms to monitor HIV/AIDS
  • Support tracing of clients on ART who interrupt ART treatment.
  • Provide community adherence counselling services for clients with high VL.
  • Support HIV/A and TB reporting using DHIS2.

Tuberculosis control program

Strategic objective 1.3.3. To increase access to and utilization of quality community-based TB control services in counties of focus.

Strategies and activities

  • Strengthen capacity of health workers in CB DOTs
  • Build capacity of community volunteers in CB DOTs
  • Support community education and mobilization on TB control
  • Support follow up and defaulter tracing in the community.
  • Support supervision and monitoring of TB services.
  • Support referral, surveillance and reporting
  • Support quarterly and annual reviews on TB services
  • Facilitate Production and distribution of IEC materials for TB control.
  • Facilitate procurement of essential equipment and supplies for TB control.
  • Support Operational Research on TB to determine its prevalence in communities.

Neglected Tropical Diseases Control

Strategic objective: To increase access to and utilization of services for prevention and control of endemic neglected tropical diseases.

Strategies and activities

  • Support Advocacy and communication activities to create awareness on NTDs
  • Build capacity of care givers to detect and treat NTDs
  • Support provision of logistics and supplies for case detection and management
  • Support integrated NTD outreaches and mobile clinics
  • Support community vitamin A supplementation and de-worming for children
  • Support supervision, coaching and mentorships for quality assurance.
  • Strengthen NTD strategic information.

Eye care services Program subcomponent:

Strategic objective: To increase access to facility and community-based eye health care services for children, women and men in programme areas.

Strategic interventions

  • Advocate for and create awareness among stakeholders and communities on the types and causes of eye diseases and promotion of eye
  • Support Community based mass campaigns for antibiotics for trachoma and prevention commodities and provision of lenses
  • Build capacity of schoolteachers in visual acuity testing and referral and outreach mass testing in schools to detect children suffering from refraction.
  • Support for policy and guidelines development for eye care service delivery.
  • Support provision and delivery of essential eye care commodities;
  • Facilitate community eye care and surgical camps for hard-to-reach communities;
  • Support capacity building for eye care workers and community eye care volunteers/networks.
  • Provide psychosocial support counseling for the blind and their families including;
  • Identification and referral of treatable blindness including cataract and corneal opacities or
  • Facilitate treatment and management of eye cases at facility and community
  • Strengthen coordination and planning for eye care services at county and state
  • Support operational research on eye care to guide program design and interventions.

Mental Health program component

Strategic objective: To increase access to facility and community based mental health services for children, women and men in counties of focus in South Sudan.

Strategies and activities

  • Support for policy and guideline development for mental health;
  • Advocate for and create awareness among stakeholders and communities on mental disorders and their manifestations.
  • Support provision and delivery of essential mental health
  • Support capacity building for mental health workers and community mental health volunteers/networks.
  • Provide psychosocial support counselling for the mentally ill and their families including workplace and off-workplace counselling for youths and adults;
  • Strengthen identification systems and referral of children with mental disorders for care in support centres
  • Strengthen systems for identification and treatment of epileptic cases;
  • Strengthen coordination and planning for mental health initiatives
  • Support conduct of operational research on mental health to guide program.
